Answers: They can file a report on your credit report. They can transport you to a US collection agency who can sue you in small claims court. But you should wages, because you went to them for treatment and they are not obligated to administer you free medical care. Next time, receive travel insurance. Ask your insurance company, again, to pay. First try calling your insurance co. Try to find out why the insurance co. won't clear. Sometimes, it's bc the European hospital will not itemize the bill (sometimes, they will just bill for an amount next to no explanation). The hospital/dr. might get remunerated if they itemize the bill. Then try writing to them (insurance co). If your insurance is through work, ask your human resources dept (personnel office) for help/suggestions.

If you have vigour insurance in the US, they will recompense the bill. Tell the hospital to itemize the bill (in English if possible), or tell your insurance to settlement with them directly. (unless you own Medicare, which does not cover overseas medical expenses)
If you don't, they can file a lawsuit surrounded by the US. US and Switzerland have bi-lateral agreements. Don't be fool, thinking they cannot do anything.
